Hi Doctor,

Sorry I write in English, I hope it's okay. I sm 29 years old and I have been on microgynon since I am 24. On my last pap smear, it was found that  I had problem with vaginal inflammation and I was treated with clindamycin and it seems to get better. The week after that I had unprotected sex with someone new and he is well endowed (big penis). We had no problem having sex as we use lubes to help making everything smooth.

However, the day after I start bleeding quite heavily. It's like I have my period and I am in the middle of my pill cycle. I am getting worried now as it has been few days and it doesn't seem to stop even though I keep on taking my pills on schedule.

I was just wondering if this is something that I need to worry about and need to get medical advice immediately or if I need to give it at least a week before getting really worried? Could it be because he is in the bigger side and it might cause some inflammation or wound inside me? I didn't feel any pain during the sex.

Thank you very much in advance for your answer.

Bleeding in connection with intercourse may be due to changes on the cervical surface (uterine neck). Especially in women on the pill, the internal mucosa of the cervix may grow to the outer surface. This ‘internal mucosa’ is rich in blood vessels and is very thin. It is named ‘cervical ectopy’. It breaks easily, such as during sex and may bleed for some time. A gynecologist can make a colposcopy (microscopy) and find out if this is the explanation. If needed, this benign condition can be treated with laser vaporization, but it often returns after some time. This is only a suggestion, a visit to your doctor for a gynecological examination will prove it, and alternative and more rare causes can be excluded

Regards from Mette

Thank you Doctor for the answer, should I go to my fastlege immediately or should I wait until the bleeding stop? I think I read somewhere that for examination it has to be "clean"?

Thank you again for your help.

You should absolutely not stop with the pill or change the type. As you did not inform me about when you had the last pap-smear, it is better to see your GP. It is better to go when you have the bleeding so the doctor can see from where it is bleeding.

Hello again Doctor,

Thank ypu for your feedback.  I had my pap smear last December. Today is my last pill before the break (I take it 42+7). So I guess i will have to bleed again in the next 7 days. I am going to see my GP on Friday morning.

Will it still be relevant then as I will be on the pill break and I will bleed as I normally would during the break?

Thank you so much again for your help.
